This page of parent resources is dedicated to making the lives of our friends and families a little more convenient. All forms must be submitted in person at the Lion & Lamb front office between the hours of 6:30am and 6:00pm. If you have any questions or concerns about any forms or submission, feel free to contact us.

Parent Permission Forms

  • All forms MUST be submitted in person by the parent or legal guardian

  • Authorization Form

    For granting of authorization for child pick-up.

  • Medication Form

    For the administering of medication to a child

  • Vacation Credit Form

    Going on vacation? Get credit for your child's time off, up to one week.

Child Registration Forms

Preschool and Kindergarten Registration is NOW OPEN for Fall 2010.
